Complete 34 Blocks to Raise Funds for Payne County!

Recruit friends, family members, coworkers or community members to help you complete your 34 donation blocks. Supporters will donate an amount equal to the block they choose. (Example: Block #1 sponsor will donate $1.) Once a block has been claimed and paid, mark off the square with that person’s name (digitally or physically).

Payments can be made through our scan-to-pay options, cash, or checks made out to United Way of Payne County. All payments should be turned into United Way by November 5, 2025. 

We invite all participants to celebrate raising money for United Way of Payne County on October 31 from 7 a.m. to 10 a.m. by walking, running, skipping, or jogging around Block 34 —— 34 times!! Participants may opt out, but are encouraged to come on October 31. 

Participants who turn in their completed Participant Sheet by 9:30 a.m. on October 31, 2025 will be entered to win a dozen Daylight Donuts EVERY WEEK for one year!

Liability and Media Release
RELEASE OF LIABILITY, ASSUMPTION OF RISK, AND HOLD HARMLESS AGREEMENT 
This Release of Liability, Assumption of Risk and hold Harmless Agreement (the “Agreement”) is executed on the below date by the undersigned, in favor of United Way of Payne County, Inc., an Oklahoma not-for-profit corporation (“UWPC”), its agencies. The undersigned hereby acknowledges that he/she has volunteered to participate in 34 @ 34 conducted by UWPC and its agencies and those entities associated with the 34 @ 34, including without limitation travel to and from the event, any travel which in any way relates to the  34 @ 34, and rescue activities. The undersigned is AWARE AND UNDERSTANDS THAT DURING SUCH PARTICIPATION IN  34 @ 34 HE/SHE MAY BE EXPOSED TO RISKS AND DANGERS INCLUDING BUT NOT LIMITED TO SERIOUS BODILY INJURY OR DEATH AND IS VOLUNTARILY PARTICIPATING IN THE ACTIVITIES WITH THE KNOWLEDGE OF THE DANGER INVOLVED AND HEREBY AGREES TO ACCEPT ANY AND ALL RISKS OF INJURY OR DEATH. As consideration for being permitted by UWPC and its agencies to participate in  34 @ 34, and as a condition precedent of the right to participate in  34 @ 34, and undersigned personally assumes all risks and damages incident to or in any way related to the event, including any and all risks or dangers caused by the negligence of UWPC, its agencies, agents, or employees. The undersigned also waives, releases and forever discharges UWPC, its agencies, agents or employees from any and all liabilities, losses, damages and costs of any nature, projects including those caused by the negligence of UWPC, its agencies, agents or employees. The undersigned further agrees not to file suit against UWPC, its agencies, agents or employees. The undersigned also agrees to indemnify and hold UWPC, its agencies, agents and employees harmless from any and all liabilities, losses, damages, costs of any nature, including those caused by the negligence of UWPC, its agencies, agents or employees. This Agreement shall be binding upon the undersigned, his/her heirs, successors, and assigns. The undersigned does further hereby grant permission to UWPC to use any photographs, videotapes, motion pictures, audio recordings, or any other record of the undersigned’s participation in  34 @ 34, as well as the undersigned’s name and/or likeness, for any lawful purpose, and waives any claim for compensation for any such use.
PHOTOGRAPH, TELEVISION, TAPE, MOVIE AND/OR SOUND RECORDING RELEASE AGREEMENT
I hereby authorize and consent that United Way of Payne County, a non-profit corporation, its legal representatives, successors or assigns, and local United Way member organizations shall have the absolute right to copyright, publish, use, sell or assign any and all photographic portraits or pictures, television spots, movie films, videotapes and/or sound recordings, or any part thereof, they have taken or made of me on this date or in which I may be included, name or other biographical data, in whole or in part, whether apart from or in connection with, illustrative or written printed matter, story or news item, motion pictures, internet usage, television or radio spots, or for publicity, advertising or any other lawful purpose whatsoever, in conjunction with my own or a fictitious name, or my real or fictitious biographical data, or in reproduction thereof in color or otherwise in any media now known or hereby created. I hereby waive all claims for any compensation for such use or for damages other than as set forth herein. I hereby waive any right that I may have to inspect and/or approve the finished product or the advertising copy that may be used in connection therewith or the use to which it may be applied. I acknowledge that there is no obligation to use any material authorized by me hereunder. I hereby warrant that I am of full age and have every right to contract in my own name in the above regard. I state further that I have read the above authorization and release, prior to execution, and that I am fully familiar with the contents thereof.
